Pop Warner Football
 The mission of Pop Warner Little Scholars, Inc. is to enable children to benefit from participation in team sports and activities in a safe and structured environment. Through this active participation, Pop Warner programs teach fundamental values, skills and knowledge that children will use throughout their lives. Pop Warner Little Scholars, Inc. (PWLS) is a non-profit organization that provides youth football programs for participants in 41 states and several countries around the world. Consisting of approximately 400,000 young people ranging from ages 5 to 16 years old, PWLS is the largest youth football program in the United States.

2012 Storm Registration

1. 2012 Physical (Required-no exceptions)
 The Association is hopeful, but does not guarantee, that doctors will be available to conduct free physicals on site. If you prefer to use your family physician, please schedule the physical appointment prior to June 30th to avoid the risk of your child being ineligible to participate in practices or games. Without a 2012 physical, your child will not practice, will not receive equipment, and will not play. This applies to returning players and new players. No exceptions! 
        Special Health Note about Heart Murmurs: If the child has a pre-existing heart murmur, please have your family provide medical clearance in order participate in tackle football. 
        Special Health Note about Eye-wear: If a player wears transitional lenses or transitional goggles, have your family eye doctor provide a medical statement indicating that the eye-wear is prescribed.

The health and safety of children is paramount. We ask that parents be proactive and include any medical clearances or documentation at the time of registration to avoid administrative delays that will hinder the child’s ability play Pop Warner Football this season. Medical Physicals dated in 2011 calendar year are void and will NOT be accepted at registration.   

2. Birth Certificate
 New Players must provide a legible copy of your child’s Birth Certificate. A military ID is also acceptable. Without a legal proof of birth date, your child will not practice, will not receive equipment, and will not play.  

3. Report Card
ALL New and returning players must provide a report card displaying 1st, 2nd, 3rd and 4th nine weeks. Home school students need to provide a copy of home school certification and a typed document showing passing grades for four full quarters in core subjects…math, language arts, science and social studies/history from the 2011-2012 school year. For all other scholastic questions, volunteers will be on site to assist you.

4. Registration Forms 
Front and back filled out completely. (Form will be posted closer to registration)

5. Parent Code of Conduct
Filled out completely and signed by parents and guardians. (Form will be posted closer to registration)

6. Volunteer Forms
(Coaches, Team Mgrs, Etc)  If you desire to become a head coach, an assistant coach, a team manager or to volunteer in any capacity. Include a valid home address, e-mail address, home, work and cell phone number. Provide a copy of your driver’s license. A background check will be performed on all potential volunteers, and you MUST include your full SS# number on form. You must provide a head-shot as well. If you don’t register in person, email or mail your photo (photograph will not be returned). (Form will be posted closer to registration)

7. Photo/Video/Interview Release Form
 Lawfully grants our league permission to display photos of your child, etc for positive and favorable purposes only. (Form will be posted closer to registration)

Cost: We accept cash or check only. Fee includes use of ALL equipment. Checks should be made payable to Jacksonville-Onslow Pop Warner Association
